Waiver

ASSUMPTION OF RISK & WAIVER OF LIABILITY

In consideration of access to the exercise equipment and recreational area provided by the facility, located at 1416 San Mateo Avenue, South San Francisco, CA 94080 I hereby expressly agree:

ASSUMPTION OF RISKS

I understand the risks of my own use of the Facility, which includes but is not limited to: the exercise
equipment, sports fields, courts, or other areas, locker rooms, sidewalks, parking lots, stairs, lobby or
other general areas of any facilities, or any equipment.

I understand the risk of my participation in any activity, class, program, instruction, or event, including
but not limited to weightlifting, walking, jogging, running, aerobic activities, tennis, basketball,
volleyball, racquetball, or any other sporting or recreational endeavor involves risk of: COVID-19,
serious injury, including permanent disability and death.

I understand and agree that the Facility is not responsible for property that is lost, stolen, or damaged
while in, on, or about the premises.

I voluntarily and knowingly assume all the fore-mentioned risks and accept full responsibility for any
injuries, damages (both economic and non-economic), and losses of any type, which may occur to me or
my property.

WAIVER OF LIABILITY

I hereby release, waiver, discharge and promise not to sue for any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated against the facility, Hydra Ventures Inc, RMA Ventures Inc, SF Elite Inc and its insurers, employees, officers, directors, associates, partners and subsidiaries from any liability resulting from any personal injury, accident, illness (including death), and property loss, however caused arising from, or in any way related to, my use and participation, my minor child(s) use and participation of the equipment or facilities, regardless of whether such injuries result, in whole or in part, from the negligence of the facility.

I HAVE READ THE ASSUMPTION OF RISK & WAIVER OF LIABILITY. I VOLUNTARILY EXECUTED THIS DOCUMENT WITH FULL UNDERSTANDING AND KNOWLEDGE OF ITS CONTENT.

I represent that I have legal capacity and authority to act on behalf of the minor named below. I hereby release and waive the rights of my child and/or children under the same terms of the Assumption of Risk & Waiver of Liability.
